Practitioner
- * Retainer based
- * Initial, in-person consult with your patient(s) and or their caregivers at your office or location of the patient's choice.
- * PRN thereafter at patient discretion.
- * All continued coaching sessions after initial consult may be in person, by phone, or virtual based on patient/caregiver preferences.
- * Your patients are guaranteed a minimum of 1 hour initial consult and a minimum of 3 subsequent, weekly, 1 hour sessions thereafter.
- * The practitioner fee covers all trauma/amputee patients and their caregivers referred by the practice.
- * Emergency Response Coaches are available to area Trauma Hospitals for immediate support to patients and their families PRN; Trauma Providers must reach out directly for this service on behalf of their patient and there is no additional fee.
- * All services to patients and caregivers must be related to the patient's trauma or amputation.
- * All patients receive a welcome gift: Core Skincare Regimen Kit courtesy of www.amputeestore.com (a $90 value)
- * All patients are entered for a chance to win a trip to the Annual, National Amputee Coalition (a three day conference which includes admission, hotel stay, and flight)

- Individual services are a highly customizable experience and will be tailored to your specific needs. One-on-one coaching can be done via email (Info@AmplitudeEndeavors.com), over the phone, or through text messaging (682-888-9376).

- * Self-pay sessions are $75/hour
- * We appreciate a 24-hour cancellation, while we do not have any hard-and-fast rules regarding this, we ask this as a courtesy to our team as well as other clients
